Medication Regimen Review (MRR) is an important and integral part of the management and monitoring of a resident’s condition. The process of MRR begins at the time of admission, and continues throughout the entire time of the resident’s stay in the NF, and is concluded upon discharge, transfer, or death. The consultant pharmacist, with the physician and nursing staff, assure the medication prescribed is safe, and appropriate to the resident’s medical condition.
Medication Administration Record (MAR) quality assurance review, ensure that the right medication is given to the resident and documentation is noted when necessary. 
Medication room/cart inspection ensures that expired medication is not overlooked, and medication is stored appropriately, while the medication pass surveys provide a good overview of the nurse’s skill to properly administer the prescribed medication according to the manufacturer and clinical practice recommendation. 

Our knowledgeable clinical Consultant Pharmacist team members provide consulting solutions for your healthcare facility team to include administration, nurses, and caregivers through personalized on-site collaboration and ongoing training and support services.

During the process, a significant number of recommendations will be made to the physician, nurse, and facility administrators. The findings are communicated electronically (HIPPA compliant) in a timely manner to the concerned parties.
On a quarterly basis, a trend analysis is provided whereby a gap is identified and analyzed. Subsequently where necessary education to the facility staff can be provided.

Interdisciplinary Approach 

During the generation of monthly trending and quality assurance reports, gap analysis is performed to identify areas of improvement such as psychoactive medication regimen, gradual dose reduction, drug-induced behavioral adverse events, health economic outcomes, and formulary evaluation. The consultant pharmacist will assist the facility staff in reviewing and if needed developing policies and procedures to ensure audit readiness as well as financial savings. Furthermore, as a key member of the interdisciplinary team, the consultant pharmacist will provide the educational services needed to the facility staff. 

Audit Support

No support is more essential than during and after an audit. Our consultant pharmacist will avail himself/herself during the audit time to answer any questions from the auditors and/or the facility staff. Our commitment is for each consultant pharmacist to personally develop a strong relationship with the facility, and to serve and support the NF during the time of audit. After the audit, the consultant pharmacist will develop lessons learned and educational material for the facility staff. 

Quarterly Review

On a quarterly basis, our consultant pharmacist will issue a report that focuses on the trends observed during the reporting period. A Pharmacist Impact Report that covers the major pertinent F-Tags of an inspection as well as a Facility Quality Review will be presented and trended from one quarter to the otherType your paragraph here.
